Your clinical home for managing everyday health, chronic conditions, and preventive wellness.
• Acute and sick visits (cold, flu, infections, minor illnesses)

Medicare: Billed per Medicare fee schedule; applicable copays and deductibles apply.
MaVie Vitality
Starts at $99/month
Personalized care designed to support energy, skin health, hair health, sexual wellness, and metabolic performance.
• Hair health and hair loss treatments
• Skin health and dermatologic treatments
• Sexual health and performance support
• Energy and fatigue support
• Metabolic and lifestyle support
• Personalized treatment plans
Visit fee includes medical services and cost of a compounded medication delivered to your home, if clinically appropriate.

MaVie Weight Health
Starts at $89/month
Medical weight management with personalized and provider-guided care.
• Personalized weight management plans
• GLP-1 and dual-agonist treatments when appropriate
• Lifestyle and nutrition guidance
• Ongoing progress monitoring
• Dose adjustments and medication management
• Lab review and metabolic screening when needed
Visit fee includes medical services and cost of a compounded medication delivered to your home, if clinically appropriate.

Insurance & Billing
Medicare: MaVie Clinic is an enrolled Medicare provider. For eligible primary care and medical services, claims are submitted on the patient's behalf. Applicable copays and deductibles are determined by the patient's Medicare plan.
Self-Pay (Uninsured or No Coverage): Patients without insurance coverage are welcome. Primary care visits are available at a flat rate of $40 per visit. Vitality and Weight Health services are available at the package rates listed above.
Medi-Cal & Commercial Insurance: MaVie Clinic does not participate in Medi-Cal or commercial insurance plans at this time. A superbill may be provided upon request; reimbursement is not guaranteed.

Mukhtar Kohistani is a California-licensed Family Nurse Practitioner and the founder of MaVie Clinic, a modern virtual practice dedicated to quality, patient-centered care. He uses a personalized, root-cause approach and secure telehealth to deliver accessible, unrushed medical care grounded in evidence-based practice and shared decision-making, empowering patients across California to take an active role in their health.
