Insurance & Payment: Medicare: MaVie Clinic accepts Medicare for covered primary care services. Applicable copays, coinsurance, and deductibles are determined by the patient's Medicare plan. Commercial Insurance & Medi-Cal: MaVie Clinic does not currently participate with commercial insurance plans or Medi-Cal. Self-Pay: Primary care visits are available for $40 per visit. Payment is due at the time of service. Superbills: A superbill may be provided upon request; reimbursement is not guaranteed.
Practice Policies: Telehealth: Patients must be physically located in California during their appointment. Prescriptions: Controlled substances (Schedule II–V) are not prescribed. Referrals: When medically appropriate, referrals to specialists will be provided. Patients are responsible for scheduling with the specialist and confirming insurance participation. Laboratory & Imaging: Laboratory testing, imaging studies, and services performed by third-party facilities are billed separately by those providers. Prior Authorizations: MaVie Clinic does not complete prior authorization requests. Cancellations: Appointments must be canceled or rescheduled at least 24 hours in advance to avoid a missed appointment fee. Refunds: Fees are non-refundable once a visit occurs or a prescription is sent to the pharmacy.
