Welcome to MaVie Clinic Professional Nursing Corporation, doing business as MaVie Clinic (“MaVie Clinic,” “we,” “us,” or “our”). By accessing or using our website, scheduling an appointment, or utilizing our telehealth services, you agree to be bound by these Terms of Service (“Terms”). These Terms govern your access to and use of our website, patient portal, and services. 1. Medical Services MaVie Clinic is a telehealth-only medical practice providing services to patients who are physically located in the State of California at the time of the encounter. All medical services are provided by licensed healthcare professionals in accordance with applicable laws, regulations, and professional standards. Use of this website alone does not establish a provider-patient relationship until required intake information and consents have been completed and a provider has performed an evaluation. 2. Not Emergency Services MaVie Clinic does not provide emergency medical care. If you are experiencing a medical emergency, you should call 911 or go to the nearest emergency department immediately. This website, messaging systems, and telehealth services must not be used for urgent or life-threatening conditions. 3. Telehealth Requirements By using our services, you acknowledge that you must be physically located in California during each telehealth visit and that telehealth has inherent limitations compared with in-person care. Certain medical conditions may require referral for in-person evaluation or higher levels of care, and services may be declined or discontinued if telehealth is not clinically appropriate or if legal requirements are not met. 4. Cash-Pay Practice MaVie Clinic operates as a private, cash-pay medical practice. We do not bill insurance, submit prior authorizations, or participate in Medicare or Medicaid (Medi-Cal). Payment is required at the time of service unless otherwise specified. Upon request, a superbill may be provided for patients who wish to seek reimbursement from their insurance provider; however, reimbursement is not guaranteed and is solely determined by the patient’s insurance plan. 5. Prescriptions and Treatment Decisions All treatment recommendations, prescriptions, laboratory orders, and referrals are based on the provider’s independent clinical judgment. MaVie Clinic does not guarantee that any medication, treatment, or service will be prescribed, and controlled substances, including Schedule II–V medications, are not prescribed through this practice. 6. Compounded Medications When clinically appropriate, prescriptions may be transmitted to licensed compounding pharmacies. Compounded medications are not FDA-approved finished drug products and are prepared for individual patients pursuant to a valid prescription. MaVie Clinic does not manufacture, dispense, store, or ship medications and is not responsible for pharmacy operations, fulfillment, shipping, availability, or pricing. 7. Patient Responsibilities You agree to provide accurate, complete, and truthful medical and personal information and to update that information as needed. You agree to follow medical instructions provided by your healthcare professional, maintain the confidentiality of your login credentials, and use services only for lawful purposes. Failure to provide accurate information may result in delayed, modified, or discontinued care. 8. Appointments and Cancellations Appointments must be canceled or rescheduled at least 24 hours in advance to avoid applicable missed appointment fees. Failure to attend a scheduled appointment or provide timely notice may result in a fee as outlined in the Financial Policy provided during the intake process. 9. Refunds Fees for medical services are generally non-refundable once a visit has occurred or medical services have been provided. Additional refund terms, if applicable, are described in the Financial Policy and other documents provided during the intake process. 10. Privacy Your use of our services is also governed by our Notice of Privacy Practices and Privacy Policy, which describe how your protected health information may be used and disclosed and your rights regarding that information. By using our services, you acknowledge that you have received or have had the opportunity to review these privacy documents. 11. Limitation of Liability To the fullest extent permitted by law, MaVie Clinic shall not be liable for any indirect, incidental, consequential, or special damages arising from the use of this website or services. Medical care involves uncertainty, and outcomes cannot be guaranteed. 12. Termination of Services MaVie Clinic reserves the right to suspend or terminate access to services at any time for misuse of services, non-payment, safety concerns, or as otherwise permitted by law. 13. Governing Law These Terms are governed by the laws of the State of California, and any disputes shall be resolved in accordance with applicable California law. 14.
